Order Your Own Lab Tests - No Prescription Required

Officially Stamped Lab Services partners with Ulta Lab Tests to provide professional blood collection services for self-pay laboratory testing.

Ulta Lab tests allows individuals to orderlaboratory tests online without insurance or a physician visit.  Officially Stamped Lab Services (OSLS) does not sell lab tests. We provide professional blood specimen collection services by appointment for patients who order their tests through Ulta Lab Tests.

HOW IT WORKS

STEPS:

1. Order your lab tests online through Ulta Lab  Tests

2. Download your lab requisition after checkout.

3.  Book you blood collection appointment with Officially Stamped Lab Services.

4.  Upload your requisition using our secure form.

5.  Have your blood drawn and sent to the lab.

6. View your results securely online through Ulta Lab Tests.

Lab Tests are purchased directly through Ulta Lab Tests. Insurance is not accepted.

Blood Collection Fees (Officially Stamped Lab Services)

On-site blood collection:  $35 (prepaid)

Mobile blood collection:  $65 (prepaid)

Collection fees are billed separately and are not included in Ulta Lab Tests pricing.

Officially Stamped Lab Services is not a laboratory and does not provide medical diagnosis, treatment, or interpretation of test results.

All laboratory testing, physician authorization, result reporting, and refunds are handled by Ulta Lab Tests.
